Section 32-6-49.9Application for commercial driver license or instruction permit; Selective Service registration.(a) An application for a commercial driver license shall be processed as provided in this section. The application for a commercial driver license or commercial driver instruction permit shall include all of the following: (1) The full name and current mailing address of the person, including his or her residential address, if different. (2) A physical description of the person including sex, height, weight, eye and hair color. (3) Date of birth. (4) The applicant's Social Security number. (5) The person's signature. (6) The person's color photograph. (7) Certifications including those required by 49 C.F.R. part 383.71 (a). (8) Any other information required by the department. (9) A signed consent to release driving record information. The application shall be accompanied by an application fee of five dollars ($5). The application fee costs for school bus drivers shall not be assessed against existing bus drivers nor school boards. (b) When the holder of a commercial driver license changes his or her name, mailing address, or residence, an application for a duplicate license shall be made as provided by law. (c) No person who has been a resident of this state for 30 days may drive a commercial motor vehicle under the authority of a commercial driver license issued by another jurisdiction. (d) Any person who knowingly falsifies information or certifications required under subsection (a) of this section is subject to suspension, revocation, or cancellation of his or her commercial driver license for a period of at least 60 consecutive days. (e)(1) Any male applicant who applies for a commercial driver license or a commercial driver instruction permit or a renewal of the license or permit who is at least 18 years of age, but less than 26 years of age, shall be registered in compliance with the requirements of Section 3 of the Military Selective Service Act, 50 App. U.S.C.A. Sec. 453, as amended. (2) The Department of Public Safety shall forward in an electronic format the necessary personal information of the applicants identified in subdivision (1) to the Selective Service System. The applicant's signature on the application shall serve as an indication that the applicant either has already registered with the Selective Service System or that he is authorizing the department to forward to the Selective Service System the necessary information for registration. The department shall notify the applicant at the time of application that his signature constitutes consent to registration with the Selective Service System, if he is not already registered.
